How well-equipped is this city for those raising infants/toddlers? Specifically, are there public places to change diapers, maintained playgrounds, etc?

Chengdu is well equipped with plenty of playgrounds and parks for toddlers, but you will not find any public places for changing diapers. Therefore, you should plan ahead before going out with a small child otherwise you will have to use a regular public restroom if they need their diaper changed.

Where can I get a list of reliable childcare providers/nannies?

One of the most effective ways to find a reliable childcare provider is through word-of-mouth. Ask other expatriates, neighbors, your landlord, co-workers and friends to see if they can recommend someone. You may also want to check some of the public bulletin boards located at supermarkets where other expatriates shop. If you do not have any luck using the suggestions above, contact your Human Resources Director or Crown Chengdu.

Discuss any safety issues for children in this city.

Chengdu is a safe environment for children, though it is recommended that you speak to your local school to find out whether there are any particular concerns specific for your area. Children should always be accompanied and supervised by an adult and should never talk to strangers. And, unlike some places, traffic is very dangerous here so it is advised you teach your children how to be cautious especially when crossing the street.
