How well-equipped is this city for those raising infants/toddlers? Specifically, are there public places to change diapers, maintained playgrounds, etc?
You can easily find kids' playgrounds wherever you go in Kansai. However, diaper-changing facilities are not very common.

Follow the traffic rules, especially traffic lights at crossing roads. Roads are narrow, with many cars and motorcycles. There are also many bicycles on the sidewalks.

Universal Studios Japan, Toys"R"Us or kids' parks at big supermarkets such as Aeon, Carrefour, Costco.
 
What are the most popular activities for kids after school or on weekends?
School organizations and sports.

There are a lot of interesting places for teens to go. Parents should be aware, however, that cigarette vending machines are everywhere and beer is also easily available.
 
Are there any organizations/social groups in the area that cater exclusively to young people ages 12-17?
Church youth groups offer a lot for this age group.

Japanese language study is a good way to learn about the culture. "Kumon" school is an after-school activity with language and math.
